St. Coletta of Wisconsin, a leader in providing services to adults with developmental disabilities, has an excellent opportunity for an experienced professional to join our Case Management team! This dynamic position works within an interdisciplinary team to provide quality services to the persons served by St. Coletta.

As a Case Coordinator with St. Coletta of Wisconsin, you will provide residential and vocational case management services to persons served while working within an interdisciplinary team. You will be responsible for the Individual Service Plan, including assessments, creation, and maintenance of a comprehensive working document, training staff on plan concepts, data compilation, and formal review. This position is also responsible for establishing individual goals and supporting strategies based on input from stakeholders, gathering information, and reporting outcome measures related to individual client growth and customer satisfaction.

Other essential functions of this position include but are not limited to:

  • Initiating and coordinating Interdisciplinary Team Meetings, as necessary, and utilizing professional meeting facilitation skills to offer a neutral, person-centered approach to reconcile team challenges and gain consensus.
  • Serving as the central communicator to external stakeholders related to program initiatives and person-served support services.
  • Serving as the central communicator with external medical personnel.
  • Preparing and maintaining written case records, reports, and forms; performing case follow-up and closing, and performing other administrative tasks, as required.
  • Attending psychiatric appointments and overseeing the behavioral health needs of residents
  • Counseling persons served, assisting them in identifying and resolving problems and making effective use of resources.
  • Advocating for the rights of persons served while educating persons served and staff on corresponding responsibilities.
  • Assisting individuals with exploring, accessing, and maintaining funding while also being responsible for ensuring the identified needs of the person served are in-line with financial reimbursement received.
  • Developing and maintaining contact with other agency staff and local, day, and other service providers to develop specialized services for persons served.
  • Identifying gaps in the service provision and works with both internal and external resources to address identified challenges.
  • Ensuring services are provided in accordance with CARF standards, funding sources and licensing agency requirements, and organizational policy and procedure.
  • Assisting in the development and communication of policy and procedures to ensure all agency regulations are adhered to.
  • Participating in organizational committees and meetings, as appropriate.
  • Adhering to St. Coletta organizational or departmental policies and procedures.

Qualifications and requirements for this position include:

  • Ability to maintain confidentiality.
  • Ability to respond to behavioral and other critical & emergency issues.
  • Demonstrated skill working with adults who have developmental disabilities.
  • Ability to establish and maintain effective professional working relationships.
  • Efficient time management and organizational skills.
  • Effective oral and written communication skills.
  • A bachelor’s degree in a related field or equivalent combination of training and experience.
  • At least one year of Case Management experience in a human service-related field is preferred
  • A valid state-issued driver’s license with a driving record that meets our requirements
